 If a dancer will be absent from a class for any reason (illness, injury, vacation, school event, etc.), a parent or guardian must complete this form, whether the absence is planned in advance or occurs the day of class. This helps ensure all instructors receive accurate attendance information before class. 

 Need another leotard, pair of tights, shoes, or required dress code item for your dancer? Submit your dancewear request & needed size through this form. Once processed, your balance due will be posted to your Studio Pro Parent Portal. Families will be notified when their order is ready for pickup in studio!
